Lockout/tagout (LOTO)

Maintain written energy-control procedures and their annual periodic inspection under OSHA 1910.147, record each live lockout with its zero-energy verification (d)(6), group members (f)(3) and release (e), with overdue inspections and unverified lockouts escalated automatically.

The LOTO module holds your written energy-control procedures, tracks the annual periodic inspection required by OSHA 1910.147(c)(6), and keeps the authorized-employee training register required by 1910.147(c)(7). It also records each live lockout — the actual application of a procedure to equipment, its zero-energy verification, group members, and release. Each procedure shows when it was last inspected and when the next inspection is due.

Add a procedure

  1. Go to Lockout/tagout and use the Add a procedure form.
  2. Choose the site and the equipment (the field autocompletes from your machine guarding list, or type it in), and add a procedure reference.
  3. Select the energy sources that must be controlled — electrical, hydraulic, pneumatic, mechanical, chemical, thermal, gravity, steam, water, or other.
  4. Select Add procedure.

Record a periodic inspection

  1. On the procedure row, choose Record inspection.
  2. Pick the inspector and the authorized employees covered, and list any deviations (one per line).
  3. Save. The next-due date advances, and the inspection history appears beneath the procedure.

Track authorized employees (c)(7)

  • Under each procedure, the Authorized employees (c)(7) row lists everyone trained to apply it, each with the date trained. Click add…, pick the worker and the trained-on date, and Save; the × on a badge removes someone who's no longer authorized.
  • A procedure with no authorized employees on file shows a yellow no authorized employees badge. This training gap only badges the row and feeds the dashboard — it does not raise an escalation notification (only the overdue periodic inspection does).

Group lockout (f)(3)

  • Tick Group lockout (f)(3) on a procedure when a crew works under it — each member affixes a personal device to a group lockout device. A group lockout badge then shows on the row.
  • Use the notes field to record the crew, lock box, or primary authorized employee.

Live lockouts — apply, verify, release (1910.147(d)–(f))

The Live lockouts card at the top of the page records each actual lockout, not just the written procedure:

  1. Apply — pick the procedure's equipment and the authorized employee applying the lockout, note the work, tick Group if a crew works under it, and click Apply lockout. The lockout opens in the Applied state, flagged amber because zero energy hasn't been verified yet.
  2. Verify zero energy (d)(6) — the critical step before any work. Click Verify zero energy, record who verified and how (e.g. operated the start control — no motion; meter read 0 V), and save. The lockout moves to Verified and the amber flag clears. Work must not begin until this is done.
  3. Group members (f)(3) — on a group lockout, each crew member affixes their personal lock (add them by name); the × removes a member's lock when they finish.
  4. Release (e) — when the work is done, click Release and record who released it. A group lockout cannot be released while any member still holds a personal lock — each must remove their own first.

Released lockouts leave the active list; the full apply → verify → release history is kept in the audit trail.

Close deviations

  • When an inspection records deviations, raise CAPA directly from that inspection to correct them; the linked CAPA badge shows once it exists.
  • When the equipment itself is gone, use Retire on the procedure — it drops out of the active list and stops escalating, but its inspection history stays.

What happens automatically

  • Procedures with an overdue inspection are flagged red in the list.
  • Escalation: a background sweep notifies the safety team about each procedure whose annual periodic inspection has lapsed, and about any active lockout whose zero-energy isolation was never verified (1910.147(d)(6)) — both link straight back to this page.
  • The dashboard counts active lockouts and lockouts still pending zero-energy verification alongside the procedure/inspection counts.
  • Every procedure's next inspection due date appears on the compliance calendar and in its .ics feed, so the annual cycle lands in whatever calendar your team already watches.
  • The dashboard counts procedures on file and inspections overdue.
  • Export register CSV (top of the page) downloads the procedure register — inspection status, group lockout, and authorized employees — from api/loto/register.csv.

Bulk load an existing register

  • Import procedures via spreadsheet import at /import — the LOTO energy-control procedures target takes SiteId (your site's name works, no codes), Equipment, ProcedureRef, and semicolon-separated EnergySources; the downloadable onboarding workbook includes a worked sheet.

Tip: the annual inspection isn't just a formality — recording the authorized employees and any deviations is what makes the procedure defensible.
