Forklifts (PIT)

The PIT module covers powered industrial trucks under OSHA 1910.178 — operator certifications, re-evaluated at least every three years per 1910.178(l)(4)(iii), and pre-use truck inspections per 1910.178(q)(7), where any defect removes the truck from service.

Certify an operator

1. In the Certify an operator form, pick the site and the operator from the roster.
2. Choose the truck classes they're evaluated on (Class I–VII) and set the evaluation date and the evaluator.
3. Click Certify operator. The certification expires three years after the evaluation date, and the operator appears in the list with a status badge — current, expiring (within 60 days), or expired — so re-evaluations don't slip past the three-year mark.

Record a pre-use inspection

1. In the Record an inspection form, pick the site, the unit (truck id), the inspector, and the shift.
2. To fail the inspection, enter one defect per line; leave the box blank to pass.
3. Click Record inspection. A failing inspection is flagged OUT OF SERVICE and lists the defects.

Out-of-service is the truck's current state — driven by its latest inspection, not by any failed inspection ever recorded. Once a truck is out of service, click Return to service on the row to record a passed re-inspection that puts it back (1910.178(p)(1)); the earlier failing row then reads "fail (since returned)".

Follow up on defects

• For a truck that's out of service, use Raise CAPA to open a corrective action to repair it. Once raised, the row links to the CAPA so you can track it to closure.

Flag an operator for refresher training

An accident, near-miss, or unsafe operation triggers refresher training and re-evaluation (1910.178(l)(4)(ii)) before the operator drives again.

1. On the operator's row, click Needs refresher…, capture the reason (accident, near miss, unsafe operation), and click Flag. The row turns red with a refresher required badge.
2. Once the refresher training and evaluation are done, click Refresher done to clear the flag.

What happens automatically

• Operator rows turn yellow when the certification is within 60 days of expiry and red once it has expired or a refresher is flagged; inspection rows that are currently out of service are highlighted red.
• Escalation: the sweep notifies the Safety Team about every truck currently out of service ("Forklift out of service: (unit)"), every operator whose 3-year certification has expired ("Forklift operator certification expired: (name)"), and every operator flagged for a refresher ("Forklift refresher evaluation required: (name)").
• The dashboard counts forklift certifications expiring within 60 days, trucks currently out of service, and a warn tile for operators flagged for a refresher — all without running a report.
• Two CSV exports: Export cert register CSV (api/pit/operators.csv) and Export inspection log CSV (api/pit/inspections.csv).

Tip: the unit field autocompletes from vehicles already in your fleet, so truck ids stay consistent across the app. The operator's certification can also seed a ForkliftOperator credential over in training.
